The healthcare team at St Mary Medical Center is always available to answer questions, address concerns and help you and your family throughout the hospital stay. Please feel free to speak with your nurse or the unit's nurse manager for assistance concerning any details during your stay
When you are admitted you will need to bring photo ID such as a drivers license, your insurance information /cards and a list of all the medications you are presently taking. Also bring the name, address and phone number of someone close to you.
Living Will and Durable Power of Attorney Upon admission, patients will be asked if they have a Living Will and/or Durable Power of Attorney for Health Care. These documents are important so hospital staff can follow your wishes should you become incapable of making decisions pertinent to your care. If you have a Living Will and/or Durable Power of Attorney for Health Care with you so a copy can be placed in your medical chart. If you have forgotten to bring your copy, please let your nurses upon admission so they may document the information and notify your physician. It is most important that you make arrangements as soon as possible to have your Living Will and/or Durable Power of Attorney for Health Care brought to the hospital to be placed in your medical record.
