What to expect

Checking in:

To check in for appointment, you will need:

  • Your license (or photo ID)
  • Your primary insurance & secondary insurance cards
  • A signed physician’s referral with an order for Nutrition Counseling and your diagnosis and diagnosis code number of it
  • A referral (if needed by your insurance company to see a specialist) 

During your individualized nutrition session, some helpful papers for your nutrition appointment include:

  • List of your current medications
  • A 24-hour food recall or your personal food journal to review 

Through Medical Nutrition Therapy (MNT), a registered and licensed dietitian will meet with you to meet your individual health goals.  Here, guidance will include setting realistic and achievable nutrition goals, creating customized meal plans, and developing lifelong strategies for healthy eating.  MNT helps individuals improve one’s overall health and quality of life.
