Based on the written orders we receive from your physician and your specific care needs, a nurse or therapist will develop a comprehensive plan of care to restore you to optimal health. We will work to adapt your healthcare needs to your home environment to maximize your independence and well-being.

nursing staff and clinical personnel are fully licensed, experienced, and highly
skilled professionals. They are selected for their dedication and commitment to
providing quality care and all are required to maintain their knowledge of the
most up-to-date treatment options through extensive continuing education and
inservice programs.
The St. Mary Home
Health Care program is part of Catholic
Health East and the Mercy Health System of Southeastern Pennsylvania and
shares the tradition of a healing ministry based on holistic, compassionate care
and service excellence and is a nonprofit organization, fully accredited by the
Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Medicare
and Medicaid Certified, and licensed by the Commonwealth of Pennsylvania.

St. Mary Home Health Care offers patients in Lower and Central Bucks County a full range of interdisciplinary skilled care. Whether itβs follow-up care after a hospitalization, short-term care following an injury at work, or care to help manage a chronic disease, Home Health Care provides convenient in-home services to meet your immediate healthcare needs. A physician referral is required in most cases and the specifics of your health situation and home environment will determine the extent of home-care services you will need. Home Health Care staff can also assist with arrangements for home medical equipment.

Skilled Nursing
Fully
licensed professional nurses provide expert care and patient and caregiver
education. Areas of specialty include :
- Cardiac Care, including Congestive Heart Failure
- Respiratory Care
- Diabetes Management

- Mom-Well Baby
Therapy All Home Health Care therapists through St. Mary Home Health Care hold a degree in their area of specialty.
- Physical Therapy β to strenghten muscles and enhance mobility to maintain the highest level of independent living
- Occupational Therapy β
to overcome impaired ability or learn to function with limited ability to
perform activities of daily living, including dressing, transferring to bed or
chair, or meal preparation.
- Speech Therapy β speech
and language training, especially with problems resulting from stroke or
injury
Medical Social Work
Medical Social Workers hold a degree in social work and are proficient in arranging alternative care options and support through community resourses.
Home Health Aides
Certified Home Health aides assist with personal hygiene, and activities of daily living
Enterostomal Nursing
Enterostomal nursing provides care for colostomies and ostomy replacement surgery, as well as wound care for hard-to-heal sores or skin ulcers.
