The Social Work Department of St. Mary Medical Center is available to assist our patients and their families to plan for their discharge from the hospital.
Services include:
Assessment of discharge planning needs
Arrangements for rehabilitation services
Arrangements for care at skilled nursing facilities and personal care homes
Arrangements for treatment at mental health and substance abuse programs
Referrals for home healthcare and durable medical equipment
Hospice services
Referrals to community agencies for services such as adult daycare, meals on wheels, homemakers, counseling, etc.
Assistance with transportation needs

Our social workers are trained to understand the feelings brought on by illness and hospitalization and are available to support our patients and their families in coping with these difficult life experiences.
Sometimes a sudden or chronic illness can affect the ability of older person to function in day-to-day activities.
The St. Mary Medical Center social work staff try to visit patients who may need services as early in their hospital stay as possible. Social workers are also available prior to admission to assist patients who are anticipating a planned hospitalization.
